A referral for someone to come and live at Community Places comes through the placing authority that is funding the placement. We ask the placing authority to provide relevant information which we will review to ensure that we are a suitable provider for the referred person.
 
We carry out a thorough assessment of the person before potentially offering him or her a place within our service. The assessment covers all health and social needs and all information is treated confidentially. Our assessment process helps ensure that we are able to meet the person’s needs and will be used as the basis of their initial personal plan, should they take up a place at Community Places.
Each assessment is carried out by a team from Community Places which includes staff members. They will arrange to meet the person and as many people centrally involved in their lives and direct care as possible, to see how Community Places can meet their needs and their wishes can be met. This is the start of the person-centred planning that ensures each person is treated as an individual.
If it is felt that our service is the right option for the person, we inform the placing authority and the transition process begins. This will be managed by a senior manager from the relevant service who will work with the person, family/carers and the placing authority to ensure the move is stress-free and smooth as possible.
